Hi There!
My name is Charlotte Slate, a licensed physical therapist who is dedicated to helping people move better, feel stronger, understand their injury, and get back to doing what they love and what brings them purpose.
A little bit about me, I received my doctorate in physical therapy from Simmons College in 2018. I have been working as a physical therapist for 7 years now. I received my orthopedic clinical specialty in 2020. I have a diverse background in treating general orthopedics, sports medicine, spine, and in 2022, I shifted my focused to an upper extremity specialty which involves the shoulder all the way down to the hand. While the upper extremity is my favorite area to treat, I am well-versed in treating the whole body and the whole person! I greatly enjoy working with athletes to help them return to their game/sport with confidence, strength, and resiliance. I have extensive experience working with baseball pitchers, gymnasts, and football players, as well as many other upper extremity injuries in various athletic populations.
I believe that healing starts first and foremost with a good foundational relationship between the therapist and client, and understanding your story! This is why I take a holistic, patient centered approach to my care, addressing not only the injury, but also your lifestyle, movement patterns, and external factors that could be contributing. Whether you are recovering from a surgery, managing a chronic condition or new injury, or just trying to feel more like yourself again, I am here to guide and support you every step of the way. INSURANCE
Slate Physical Therapy is an out of network provider, therefore I do not accept insurance. Payments will be collected at time of service. You will be provided with the necessary documentation, a "Superbill", to provide to your insurance company for reimbursement. Before your visit, I recommend you call your insurance company and ask the following:

How can I submit out-of-network PT documentation (a “Superbill”) for reimbursement?
Payments are accepted via cash, Venmo, check, credit card, or FSA/HSA.
When receiving PT in a traditional insurance model, you pay a co-pay for each visit. On average, the co-pay can range from $30 to $75 per session. In this model of PT, you are usually seen 1-2 times per week with 30-minute sessions. You will be required to pay a copay each visit, so your out-of-pocket payments can add up quickly! In the cash-based PT model, you pay a flat fee per visit, and I will be able to provide you with one-on-one individualized care that is focused on you without the constraints that insurance puts on us. I recognize that it may feel like a significant investment to work with me. I would love to discuss this further with you and answer any questions that you may have!
